Terms and Conditions for Membership Application of Senior Member
1. All elderly doctors above the age of 70 years can apply for
senior membership. Elderly doctors not having MMA membership can
also apply.
2. For membership, please fill the senior doctor application form (SGED
-01) together with 2 passport size ( 1" x 11/2" ) photo and submit
to secretarial office.
3. For elderly doctor who cannot come to the office, can make phone
call to the office staff and they will arrange to collect data at
homes for convenience.
4. They are not liable to pay membership fees. (Senior membership is
free of charge)
5. Once they obtain membership, it is for life long and no need to
do extension .But please inform to the office if there is any
changes in data (e.g. change of address, phone, etc.)
6. If they need any kind of help or assistance, they can make a
phone call to the office (phone no. 394229) during office hour. If
it is after office hour, they can inform the following EC member by
phone.

Terms and Conditions for Donor club of SGED
1. Whoever (medical or non-medical) voluntarily wants to support
elderly senior doctor can apply SGED donor club.
2. All they have to do is fill the donor club application form (SGED
-03) together with 2 passport size ( 1" x 11/2" ) photo and submit
to secretarial office.
3. The membership categorizes and fees are as follows.

4. The membership period is one year from the date of application.
After one year period, application for membership extension is
required. SGED secretarial office will inform the member one month
prior to expiration date.
5. Office will issue membership card two times a month. If the
application form is between the 1st -15th days of every month, the
card will issue after 25th day of that month and also for
application between 16th - 31st days, the card will issue after 10th
day of next month.
6. The membership cards will be issued at office personally or any
authorized persons on behalf of the member.
